The priority of SCH is providing high quality, reliable and safe community healthcare to people in the community, delivered with compassion and kindness.
The 15 community health teams offer nursing and therapy care, working with specialist nurses. SCH also operates four community hospitals and a minor injuries unit, and a wheelchair and community equipment service to help people maintain their independence for longer.
The community paediatric service offers an integrated service to children and young people with disabilities and longer-term health conditions.
